Job summary
HHFT is recruiting for a new innovative service and are looking for enthusiastic, patient centred individuals.
The service aims to support patients through early supported discharge from hospital, and work collaboratively across primary and secondary care to avoid hospital admissions by spotting deterioration earlier in patients with long term conditions.
The Virtual Health will be sitting in the heart of the urgent and emergency clinical environment at HHFT as part of a dynamic multi-disciplinary team.
The role of the Virtual Health Hub Administrator will be working with a skilled team of Nurses, Pharmacist, Allied Health Professionals and Doctors by providing administrative support. This will involve liaising with community teams and enabling service users to speak to the right person at the right place at the right time.
Main duties of the job
Responsibility for providing administrative service to the Virtual Health Hub. Including receiving and dealing with calls, emails, correspondence and other contacts from service users and handling these as required, in order to coordinate care according to agreed protocols and clinical pathways.
To understand and be familiar clinical documentation protocols when receiving referrals and handing over clinical information to clinicians.
Preparation and management of referrals by ensuring that follow up actions from calls, emails, correspondence and other contacts are progressed efficiently and effectively.
Responding to urgent requests as necessary and ensuring that urgent and sensitive items are dealt with accordingly.
To facilitate with the smooth running of the administrative services by ensuring that the working environment is kept tidy, safe and secure.
