Job summary

The post offers experience in management of acutely unwell patients, frailty, dementia, delirium, as well as long term conditions. There will also be opportunities to lead Multi-Disciplinary Team meetings and to be involved in teaching, management, and governance.

Main duties of the job

• Leading the ward-based junior medical team under supervision of the Consultant.
• Review and presentation of new admissions/transfers to the
• Initial management of new admissions until Consultant review, and referral to other specialties as clinically indicated.
• Contributing to ward MDT meetings to aid discharge
• Liaising with family members, undertaking discussions about end-of-life care where necessary, and engaging with the palliative care team
• Assessment of new patients in frailty same day emergency care under the supervision of consultants
• Out-patient clinics
• Maintenance of CPD, and the teaching and training of
• Attendance and contribution to departmental teaching
• Demonstration of the necessary communication skills required to support the above responsibilities, in line with GMC
• Understanding of the Mental health Act as it relates to capacity decisions etc (training will also be provided as part of the induction to the trust).
