Medicines Management
To work alongside Clinical Pharmacists supporting the Medicines Management service to the wards, to ensure patients receive the highest standards of care in the management of their medication. Duties will include medicines reconciliation, assessment of patients own drugs (POD's), checking of medication for discharge and the counselling of patients.
To be involved in developing and delivering enhanced medicines management services and training.
Antimicrobial Management
The post holder will work closely with the lead pharmacists for antimicrobials and the lead microbiology consultant for antimicrobial stewardship to develop, innovate and strengthen the antimicrobial stewardship strategy and culture within the trust. This post will have scope to develop elements to the post holder's areas of interest that contribute to the overall stewardship strategy.
Main duties of the job
Working in collaboration with the lead pharmacist for antimicrobials and the lead consultant for antimicrobial stewardship with data to inform antimicrobial stewardship strategies and activities across the trust..
o Working in collaboration with the lead pharmacist for antimicrobials and the lead consultant forantimicrobial stewardship with data to inform antimicrobial stewardship strategies and activities acrossthe trust.o Contribute to a culture of continuous learning around healthcare acquired infection (HCAI) andantimicrobial related incidents around the trust through route cause analysis and stewardshipinterventions in periods of increased HCAI incidence.o To take a key role in the use, development and maintenance of IT infrastructure supportingantimicrobial stewardship strategy. (ICNet, MicroGuide) To lead on patient centric stewardship activitysuch as specialist antimicrobial counselling on discharge.o To liaise with the outpatient antimicrobial therapy (OPAT) team and the patients cared for by thisservice and support the medicines management within this new service.
