Clinical Nurse Specialist (Acute Kidney Injury and Sepsis)

Job summary

We are seeking a proactive and highly motivated Band 6 AKI & Sepsis Nurse to join our Adult Sepsis and Acute Kidney (ASK) Team. This specialist role supports the early identification, assessment, and management of patients with Acute Kidney Injury and Sepsis across the organisation. You will work autonomously across front-door and inpatient areas, delivering expert clinical assessment, initiating timely interventions, and guiding teams in the use of the SHOUT bundle and Sepsis Six.

You will educate and support staff in recognising deterioration, contribute to quality improvement initiatives, and promote best practice in line with NICE guidance. The role involves close multidisciplinary collaboration, providing expert advice on fluid management, medications, and treatment planning. You will also support safe discharge planning, including telephone follow-ups for AKI patients, and play a key part in delivering a reliable seven-day service.

Main duties of the job

Provide specialist clinical assessment and management for patients with Acute Kidney Injury, ensuring timely identification, treatment using the SHOUT bundle. Work proactively across front-door areas and inpatient wards to identify deteriorating patients, initiate appropriate interventions, and ensure prompt senior reviews. Support staff in the early recognition of sepsis using the Sepsis Six tool, guiding clinical teams to deliver timely treatment in line with NICE guidance. Deliver high-quality education and training to multidisciplinary teams on AKI prevention, management, sepsis recognition, and treatment. Lead and contribute to quality improvement projects relating to AKI and sepsis care .Work collaboratively with ward teams, the Emergency Department, and specialty services to enhance early intervention and integrated care. Provide expert advice on fluid management, medication, and treatment plans for patients with or at risk of AKI. Support the delivery of a seven-day service, ensuring equitable specialist cover, continuity of care, and timely patient review. Guide safe discharge planning and follow-up for complex AKI patients, including telephone follow-ups to support recovery and prevent readmission. Act as a clinical role model, demonstrating advanced nursing practice, patient advocacy, and high standards of care. Participate in policy development, pathway refinement, and implementation of NICE-aligned best practice for AKI and sepsis management.

  • Teaching and Assessing Qualification/ ENB 998/ City and Guilds 730 or equivalent
  • Bachelors Degree (or evidence of level 3 learning supplemented by diploma level specialist training/experience/ education)

Desirable

  • Relevant clinical course to enable history taking, and clinical examination, diagnostic reasoning, and differential diagnosis

Experience

  • Demonstrable post qualification experience within an acute environment
  • High standard of nursing care and clinical knowledge
  • Ability to support patients and family members

  • Computer Literacy
