Job summary
This is an exciting opportunity to work within a dedicated, specialist team, caring for people diagnosed with Crohn's disease and ulcerative colitis, the two main types of Inflammatory Bowel Disease (IBD).
Working as a member of Inflammatory Bowel Disease (IBD) Team you will provide support and administrative duties in the delivery of services for patients with Inflammatory Bowel Disease making a direct and positive contribution to the work of the IBD team. As a pivotal member of the IBD service you will act as an initial contact for primary and secondary care to the IBD service via telephone and email.
A key part of the role will be collating data and maintaining databases including those with a new diagnosis and patients starting new treatments, helping to ensure a smooth and efficient pathway for patients.
You will also manage the co-ordination and organisation of the IBD Multi-Disciplinary Team meetings (MDT) supported by the lead IBD CNS and IBD clinical lead. Recording the clinical outcomes on the patient records and the follow up of decisions taken.
Having the ability to exercise initiative and judgement when dealing with patient enquires, requests, referrals and results, is an important part of the role along with prioritising the importance accordingly.
Main duties of the job
The IBD coordinator is a vital role within the IBD team. You will organise patient care, coordinate the multidisciplinary team (MDT) meetings, manage patient queries, and ensure smooth patient pathways for those with Crohn's disease or ulcerative colitis, bridging gaps between patients, clinicians and data systems.
