Job summary

Are you an enthusiastic and forward-thinking Pharmacy Technician looking to take the next step in your career? The Great Western Hospitals NHS Foundation Trust is seeking a motivated Medicines Optimisation Pharmacy Technician to join our dynamic and supportive clinical pharmacy team.

In this role, you will play a key part in ensuring safe, effective, and efficient use of medicines across our hospital. Working closely with pharmacists, nursing staff, prescribers and wider clinical teams, you will support high-quality patient care by delivering accurate medicines reconciliation, improving medicines safety, and helping patients get the most from their medicines.

Job responsibilities

  • Carrying out timely and accurate medicines reconciliation for patients on admission.

  • Supporting safe and effective discharge through processing discharges at ward level and counselling patients about their medicines.

  • Participating in ward-based medicines optimisation initiatives to reduce medication errors and improve outcomes.

  • Helping implement best practice, formularies, and medicines policies across the Trust.

  • Supporting medicines supply, stock management, and minimising waste.

  • Using digital systems to improve the flow of medicines information and support clinical decision making.

  • Acting as a role model for high-quality, patient-centred medicines management.

Were looking for someone who is:

    • A registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).

    • Experienced in hospital or primary care pharmacy (though we welcome motivated candidates looking to broaden their experience).

    • Confident communicating with patients and multidisciplinary teams.

  • oKnowledge of medicines management, including relevant legislation and policies acquired through NVQ3 or BTEC equivalent level qualification plus specialist training and experience to degree or equivalent level
