Job summary
An exciting opportunity has arisen to join the Older Persons Community Mental Health Team in Great Yarmouth.
We are a friendly, supportive, and experienced multidisciplinary team committed to delivering high-quality, evidence-based, person-centred care to older adults, typically aged 75+, experiencing functional mental health difficulties and those of any age experiencing problems associated with Dementia.
This role is ideal for an experienced Band 6 practitioner or a Band 5 looking to progress into a more senior role . We are passionate about supporting staff to grow and will provide the guidance, supervision, and training needed to help you succeed in the role.
Previous experience of working with older adults is not essential as you will receive a tailored induction programme.

Main duties of the job
The post holder will:
- Work as a Care Coordinator for a caseload of service users with complex mental health needs
- Deliver person-centred, evidence-based interventions to older people and their carers
- Undertake comprehensive assessments, including risk formulation and safety planning
- Liaise effectively with a range of professionals and services to support holistic care delivery
- Participate in daily Safety Huddles and weekly MDT meetings
- Contribute to the shared Duty Worker role within the team
- Triage referrals into the service
- Maintain accurate and timely clinical records using electronic systems
- Support and mentor students and colleagues as appropriate
Caseloads are carefully managed using a caseload weighting toolkit to ensure they remain safe, balanced, and sustainable, supporting staff wellbeing.

- Undertake initial, and ongoing patient assessments, including risk assessment and screening.
- Continuously review and evaluate patients needs, supporting safe and timely discharge planning in collaboration with senior colleagues.
- Enable and support patients to maintain and maximise their independence wherever possible.
- Promote the active involvement of patients and carers in care planning, ensuring they are informed, empowered, and central to decision-making.

- Communicate any changes in patients conditions or circumstances promptly to the wider team, including medical and social care professionals.
Leadership and Supervision
- Provide guidance, support, and supervision to junior staff as delegated by the Senior Nurse.
