Job summary
Norfolk and Suffolk Foundation Trust is seeking a compassionate and skilled Veterans Mental Health Practitioner. This post will involve working within the collaborative provision of the Op Courage Veterans Mental Health Service which is run by the Essex Partnership University NHS Foundation Trust (EPUT). This service covers the six counties that fall within the geographical patch of the East of England.
The post holder will be responsible for the care of a caseload of patients under the Op Courage Community Veterans Mental Health Service. You will work as part of a multidisciplinary team to provide assessment, care planning, monitoring of mental health, and risk assessment of clients referred to the service. You will work as part of the MDT and will present cases to aid with clinical decision making around treatment, care and support of the Veteran. You will monitor the care and treatment of the Veteran with the aim of working towards discharge from the service.

Main duties of the job
- Manage a community caseload, completing assessments, coordinating care plans, and supporting safe discharge.
- Build therapeutic relationships and understand the specific needs and experiences of the Armed Forces Community.
- Identify mental health needs and risks, developing person-centred, needs-led care plans.
- Refer to appropriate services and liaise with statutory, non-statutory, and military-related organisations.
- Present cases to the MDT and maintain high-quality clinical records in line with KPIs.
- Contribute to incident reporting and service improvement, ensuring practice aligns with current veteran mental health policy.
- Maintain up-to-date knowledge of local support, charities, and community resources to empower individuals in their recovery.
