Senior Community Mental Health Nurse

Job summary

This is an exciting opportunity to join a passionate and motivated, multi-disciplinary team of professionals who support people with dementia and their carers to remain living independently in the community.The team has a philosophy of early intervention and specialises in working with those who are hard to engage .

We become involved whensituations are breaking down. The team aims to prevent unnecessary admissions to hospital and premature moves intolong term care. There is an open referral system , and we work with people for time limited periods, aiming to stabilise situationsand ensure there is support in place to meet their needs in the future.

The team has been focussed on quality improvement for both service users, carers and the staff team, so this is an opportunity to be a part of an improvement initiative. As a senior practitioner, you will help to shape the future of the service and assist in its leadership.

Main duties of the job

Working with other multi-disciplinary professionals within the team you will:o Manage and hold a caseloado Undertake initial assessments including risk assessmentso Engage and work with clients with dementia and their carerso Liaise with statutory and voluntary Sector to organise care and support as requiredo Triage referrals and undertake duty roleo Attend Clinical and Business Meetings for the teamo Take on a supervisory role within the teamo Represent team in meetings with partner agencies, including INT meetings.
