Job summary

This is an exciting opportunity for existing clinicians or newly qualified mental health nurses/social workers/occupational therapists to join the West Youth Community Mental Health team based at Thurlow House in Kings Lynn as a Band 5 Mental Health Practitioner. This is a full time (37.5 Hours) post, working with service users ages 14-25 years of age.

You will be working in highly skilled multi-disciplinary team with a strong team ethos of supporting each other. You will be supported to develop a strong understanding of your role as a Care Coordinator/Assessor becoming skilled at delivering evidence-based treatment interventions for our service users. Working in partnership with other agencies will be a big part of the job so good communications skills will be vital.

Main duties of the job

Day to day the role will involve core duties some of which are listed below:

You will enjoy plenty of opportunities for joint working, training, and a sound supervision structure.

You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never far away if needed.

The ideal candidate should have experience in working in a mental health environment and ideally experience in working in community teams.

Working in a busy and dynamic Youth Community Mental Health Team means every day will be different:

  • Managing a caseload via face to face, online and telephone contacts
  • Waiting List monitoring via telephone calls
  • Undertaking initial assessments on behalf of the service
  • Undertaking face to face visits in the community setting including home visits
  • Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
  • Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as Social Care, Crisis Resolution and Home Treatment Team, U18 Crisis Team CAIST, Adult Community Mental Health Teams, MHT, Acute Hospitals A&E, and third sector organisations)
  • Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm) may be part of this role moving forward
