Clinical Nurse Specialist (Older Person Community Mental Health)

Job summary

We are looking for a dynamic registered practitioner with advanced clinical skills. We want applicants who are keen to be part of transformation and improvement in the area of Older Persons Community Mental Health services. We require people who can offer strong clinical leadership, education and mentoring to the team.

Central Norfolk Older Persons Community Mental Health teams provide a Monday to Friday 9am to 5pm service including assessments and treatment pathways. You will be responsible for supporting the team with triage, providing advice to primary care clinicians where appropriate, You will be required to prescribe and your scope of prescribing practice must include prescribing in Older Persons Mental Health. You will also be required to support the wider team with assessments and treatment planning, aiding the team with risk management and co-ordination of care delivery within a well-resourced team. You will be supported with regular supervision and opportunities to work in partnership with other health and social care providers, linking into the wider multi-disciplinary service.

Your role within the community team will include:

  • Caseload management, supporting with the provision of mental health care services
  • Supporting and advising other health care professionals within the wider team
  • Giving advice about risk and demonstrating comprehensive assessment skills
  • A positive role model and mentor to colleagues and junior members of staff
  • Providing appropriate interventions to patients with mental illness (incorporating NICE guidelines)
  • Initiate, create and maintain therapeutic relationships with patients and carers, ensuring that practice is patient centred.
  • Supporting and develop the principles of joint partnership working and to be pro-acting in fostering positive interagency relationships and information sharing
