Job summary
An exciting opportunity has arisen for a Social Support Coordinator role working across our Crisis Resolution Home Treatment Team (CRHT), Community Mental Health Team (CMHT) and Samphire Ward in West Norfolk.
This post will assist teams to maintain a focus on patient's holistic needs and social recovery, offering social prescribing, advocacy, navigational signposting and coordination of social needs.
Using a partnership approach and making full use of external agency support where appropriate to provide access to relevant advice and long-term support networks for clients.
The post will act as a key stakeholder and advocate for the voluntary and social care sector, promoting the needs of patients accessing acute mental health care
Main duties of the job
Assess client's needs and establish rapport to produce an agreed support plan. Where possible promote a recovery model that empowers clients to be involved in decision making and support tasks.
Breakdown barriers and facilitating safe discharge with patient-centred support in the community. Point of contact for social care/housing/agencies for coordination of safe discharge. Robust multiagency working with Statutory and Voluntary services to promote independence and recovery within the community.
Provide practical help and emotional support to service users when required, including intensive support to individuals in a timely, effective, and integrated manner.
Work with third party and statutory bodies and establish community links/support to enable clients to access service provision including Social Care, Housing Services, Police and Courts, etc.
Assess and provide advocacy to enable client's financial position to be optimised. This would include signposting/referral for budgeting & debt services including Local Authority services.

Provide housing advice and referral via a Duty to Refer to Local Authorities under the Homeless Reduction Act, for people at risk of or Homeless. Support to secure alternative housing where present accommodation may be unsuitable for clients mental or physical needs.
