TheGreat Yarmouth Primary CareNetworkis made up of thefollowing GeneralPractices;Lighthouse medical centre, Park Surgery, Newtown Surgery,and the Coastal Villages surgeries.

You'llbe working alongside an experienced lead mental health practitioner.We'rea collaborative team and work closely with all those who are based at the practices.There will be a period of induction and orientation shadowing other PCN MHP and there are regular opportunities to access relevant training.

Our Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of an integrated health and care system, which brings care closer to home for our population.

Main duties of the job

As a PCN MH practitioner, you will work across the interface between Primary Care and Secondary Mental Health Services undertaking the planning and delivery of direct patient care.

The ideal candidate will have an interest and experience in a mental health setting;you'llhave good organisational skills as you willneed to manage your own workload, as well as offering support and advice to other health care professionals. You will be an autonomous practitioner, making clinical judgements,identifyingalternative courses ofactionand providing high-quality care.

We'recommitted to training, continuous professional development and ensuring all members of the team can access resources to help them to develop and progress. When you join us, you can expect to be supported from the start;you'llspend time with our community teams learning about thedifferent rolesthey have, as well as shadowing other members of the PCN.
