Our Memory Assessment Team - Care Homes (MAT-CH) works with our service users to help recovery and make a positive difference to their health and wellbeing. This service covers care homes across Norfolk & Waveney focussing on dementia diagnostic assessment, care planning and working with care home staff members in managing service users' needs and challenging behaviour. We believe that working closely with our service users and their family or carer allows us to provide bespoke care planning to meet their needs.
The post holder will be visiting service users in the care homes, assessing their mental health needs. The post holder will have an identified NSFT base and there is also an opportunity for hybrid working.
We're looking for individuals who have particular experience in working with older people community mental health services especially dementia assessment and management. The post holder will work closely with wider older people community mental health services, sharing experience and knowledge to continually learn from each other.
Main duties of the job
Older peoples nursing can be overlooked, however the complexity of the skills required for care for a patient with dementia embodies what mental health care is about and pulls on every facet of our training and knowledge base.
Memory Assessment Team - Care Homes (MAT-CH) is mostly nurse led service which links with locality consultant colleagues for complex case interventions. You will be become an essential part of our multi-disciplinary team of Nurse Consultant, Service Manager, Clinical Nurse Specialist, Advanced Nurse Practitioner, Non-Medical Prescribers and Community mental health nurses, supported by our fantastic admin team.
As a team we feel it is vital to raise awareness around the need for more research, early intervention and sign posting, and you will play a vital role in this, using your existing expertise and supported with continuous learning from the supportive team.
This diverse range of specialities in the team allows us to work collaboratively with the aim of providing high quality and integrated care.
Please note that this is a community-based role, the successful candidate will need to have a full UK driving licence and access to their own car.
