There is an exciting opportunity to join the North Older Person's Community Mental Health Team. Our base is located close to Norwich City Centre at the Julian Hospital. Our Team is rated good by the CQC and we have recent received a Research Impact Award, at the last Trust awards ceremony. We have a long history of excellent staff retention. We have adopted a compassionate leadership style and have a culture of coaching and supporting our staff to deliver a high quality of care. Staff will receive regular line management supervision and time to discuss their caseloads and their wellbeing. We have one day per month that is dedicated to attending our monthly governance meeting, internal and external training and CPD. Main duties of the job
The successful candidate will be working with service users with Dementia or suspected Dementia, functional mental health conditions, such as with people on the SMI register & depression and anxiety. You will work alongside their family, carers and other service providers involved in their care.
You will be responsible for the planning, review and delivery of care and treatment in the community in conjunction with the service users on your caseload. Usual caseload numbers are around 20-30 services users per full time staff.
You will provide duty cover for the team on a rostered basis and will support individuals in need and will liaise with other agencies to ensure care needs are met.
Youwill require excellent communication and organisational skills and the ability to work flexibly on a busy caseload. Assist with the supervision of peers if requested. Enable patients to gain and maintain independence. Identifying personal learning and training needs through KSF, appraisal and personal development plans on an annual basis. Undertake the supervision of students that will be undertaking placements within the Team.
Working hours are currently 9am- 5pm Monday to Friday. The successful candidate will have to opportunity to work flexibility as per the NSFT flexible working policy. You will be able to work a mixture of home working, working from base and seeing patient's in their own home. There is free parking for staff and services users on site.
