Our Community Mental Health Team covers the West side of Norfolk, we support a range of older people who are living with a wide range of Mental Health concerns that may or may not include a dementia diagnosis.
Our team work together to support each other as well as giving you the autonomy to work with individuals, develop a therapeutic relationship and support individuals to live as independent and fulfilling life as possible.
Main duties of the job
You will be part of an effective and mutually supportive clinical team, with strong support from the management team.
Your main role will be supporting and enabling the Memory Assessment & Treatment Service at Chatterton House, obtaining patient information and providing some follow up either at Chatterton House or in the Community setting, this will be with supervision from Nursing professionals, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
