Are you an experienced Mental Health Practitioner looking to work autonomously, in a forward thinking, fast-paced environment?
Whether you are a Registered Nurse, Occupational Therapist or Social Worker; this is the ideal challenge for someone who is looking to make a difference to the local community of North Norfolk.
You'll be working alongside an experienced lead mental health practitioner, as well as alongside Mental Health Practitioners based at the different surgeries. We're a collaborative team and work closely with all those who are based at the practices.
Our Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of an integrated health and care system, which brings care closer to home for our population.
The North Norfolk (NN3) Team are passionate about developing their mental health provision; they're proactive and enthusiastic and need a forward-thinking, driven mental health practitioner to help them achieve their aspirations.
NSFT and North Norfolk Primary Care (PCNs) have come together in partnership to embrace integrated mental health care delivery for our local population. The North Norfolk Primary Care Network (NN3) have 2 individual practices to cover; Drayton & Coltishall

Main duties of the job
As a PCN MH practitioner, you will work across the interface between Primary Care and Secondary Mental Health Services undertaking the planning and delivery of direct patient care.
The ideal candidate will have an interest and experience in a mental health setting; you'll have good organisational skills as you will be managing a caseload, as well as offering support and advice to other health care professionals. You will be an autonomous practitioner, making clinical judgements, identifying alternative courses of action, managing and providing high-quality care.

When you join us, you can expect to be supported from the start; you'll spend time with our community teams learning about the different roles they have, as well as shadowing other members of the PCN.
