Job summary

Clinical Support Worker - Mother and Baby Outreach team

An excellent opportunity has arisen to join the Mother and Baby Outreach service. You will be joining a motivated and passionate multi-disciplinary team working to achieve positive outcomes for service users and their families.

The team is mainly community based and works in individuals homes to prevent admission to MBU or to support discharge from the MBU, there will be some requirement to get to know service users while they remain as an inpatient to build therapeutic relationships that will support discharge.

The role involves you working with service users, building trust and rapport and enabling them to have the appropriate care and support in place to continue on their recovery journey.

The role involves working with the support of practitioners but managing your own workload and working autonomously in the community.

Main duties of the job

To undertake work as guided by Practitioners with a specific and clear role from the outset.

To build relationships and engage with individuals and their family

To provide intensive short-term goal orientated input to the person

To explore and better understand needs and risks for individuals

To support people to access resources in the local community

To demonstrate/transfer knowledge and skills to other carers/family members

To be flexible and adaptable in the input you give

To highlight any concerns or risks to the Practitioner responsible for the case

To write notes of visits and input onto the computer system in a timely manner

To be able to manage own workload effectively

The MBU Outreach team Team is a part of the Specialist Services Care Group, working within the community to meet the needs of people with perinatal mental health needs.
