Clinical Nurse Specialist - Community Mental Health Team - City 2

We are excited to have an opportunity for a professional practitioner to join our established team.
You must be able to demonstrate a warm, compassionate and needs-led approach to working with our service users.
As part of this team, responsibilities would include undertaking assessments, working collaboratively with individuals and other supporting agencies to manage needs, as well as holding a caseload.
Main duties of the job
This role will give you the time and facilities to make a difference. You will have the ability to offer person-centred care while building trusting relationships to increase engagement with individuals suffering from severe mental illness in the community. The main duties are:
- Assessing patients mental state and formulating working diagnoses
- Prescribing appropriate pharmacological and non-pharmacological treatment plans for a range of mental health conditions such as psychotic disorders, affective disorders, insomnia, personality disorders and anxiety disorders.
- Monitoring the effectiveness of treatment
- Writing of care plans and discharge letters
- Basic interpretation of blood results
- Clinical leadership for the staff within the community team
In turn we will provide you with regular clinical and management supervision with an emphasis on reflective practice. We hold a strong belief in team and individual development.
