Job summary

An exciting opportunity has arisen for a full-time band 6 mental health nurse within the Older People team in Ipswich.

We are looking for a nurse who can work independently and as a part of a team, is forward thinking, highly motivated and keen to work with older adults and their carers with a person-centred and holistic approach.

You will be part of psychologically minded and highly performing multidisciplinary team of clinicians. You will be delivering biopsychosocial interventions under the Recovery Model. You will work with older adults with dementia and functional conditions.

We promote hybrid working. Professional meetings, telephone contacts and administration work can be completed remotely but we require office attendance to cover duty work, weekly MDT meeting and face to face appointments for the service users who require it.

Main duties of the job

As a Community Mental Health Nurse you will be responsible for:

  • Holding a caseload of patients with organic and functional presentations under CPA and NCPA
  • Assessing and managing risk.
  • Using Dialog+ where possible and Recovery Care Planning with service users and carers.
  • Managing your own diary and planning your day.
  • Attending MDT meetings and ward review meetings.
  • Duty working one day per week.
  • Communicate effectively through notes on Lorenzo and letters to GPs and other involved parties.
  • Administering depot medication.
  • Driving is essential for this role.
