Job summary

We are pleased to offer the opportunity for an Assistant Practitioner to join the North Norfolk and Norwich care group in the Memory Assessment and Treatment Team.

The team is providing assessment and treatment for people who have noticed problems with their memory and may be at risk of having developed dementia.

We aspire to be ranked among the best Memory Services and are on the Developmental Pathway under the Memory Services National Accreditation Programme (MSNAP). We are looking for staff who share our aspirations and want to provide a great service to people in Norfolk.

Main duties of the job

This role supports our Memory Assessment and Treatment Team and the appointed person will work with patients who are referred for a memory assessment to provide support and also gather valuable information to help the memory assessment process.

The role will also support patients and families after they have received a diagnosis for their memory difficulty, both individually and in groups, which will include running Cognitive Stimulation Therapy Groups and Carers' Groups and they will receive appropriate training for this.
