CATAThave been recently commissioned to support Children and Young People (CYP) under the age of 18. The aim of CATAT is to prevent CYP from having inpatient admissions wherever possible, and to support them and their family in the community. The team will serve as a gateway in and out of Tier 4 (T4) provision so CYP will be able to access T4 provision / support without the need for admission. This is a significant change in both the clinical offer and culture.
CATAT are also looking at post discharge support as evidence shows that CYP in the first 5-7 days post discharge may be more likely to re-engage in risk behaviours, which can potentially result in a re admission or attendance at an acute hospital. With this in mind CATAT are looking at it as a preventative rather than a reactive intervention to enable admission avoidance. Using the Ambit & Thrive model this adds to the total offer and supports stabilisation and of the system around the child.
We care for CYP across Norfolk and Suffolk so will have to be willing to travel ( we do car share at times/ take it in turns to drive)
Main duties of the job
CATAT work alongside the core community teams as well as partner agencies to offer young people and children that require additional support and therapeutic input, this allows the community-based services to offer input to all children and young people referred to their services. CATAT are not offering a crisis service, rather a model of a prevention as opposed to reactive when there is a crisis. In a Longitudinal examination of youth readmission to mental health inpatient units by Miller D et al 2020, evidence offered was that-
"In total, 27.8% of admitted children and adolescents experienced at least one readmission within the 10-year period, with 57.3% readmitted to hospital within 90 days following discharge".

You will be involved in enhanced assessments, risk management, involved in meetings such MDTs, CETRs, LAEPs, care planning, and co-ordination of care delivery. We in CATAT used STORM, Ambit and Thrive models to guide us the interventions we use for our CYP.
