GP Intermediate Care/Hospital at Home - Various part time roles available, minimum 4PAs

Are you a GP passionate about high-quality, person-centred care? Do you enjoy multidisciplinary working and want to shape community-based medical services? We are expanding our Intermediate Tier Services and seeking motivated GPs to join our Intermediate Care and Hospital at Home teams.

This is an opportunity to contribute to a developing GP-led model focused on excellent care for older people, strong clinical leadership and integrated community pathways. Roles are part-time, with flexible PAs agreed at interview.

The Stamford Unit provides intermediate care beds offering rehabilitation, end-of-life care, step-up admissions and complex discharge planning. You will lead the medical care for a cohort of patients, undertake ward rounds and work closely with ACPs, Nurses, Therapists and Social Workers to deliver safe, holistic care.

Hospital at Home delivers acute-level care in patients' homes as an alternative to admission. Working with Urgent Crisis Response teams, advanced nurse and paramedic assessors and specialist respiratory and heart-failure services, you will complete telemedicine assessments and support coordinated, responsive care planning.

This varied role includes both bed-based and community acute work, with the chance to develop interests in older people's medicine and integrated care. A 1:4 weekend rota provides cover across the Stamford Unit and Hospital at Home services.

Main duties of the job

  • Provide senior clinical leadership across Intermediate Care and Hospital at Home services, working with Divisional and Directorate leaders to ensure safe, high-quality care is delivered seven days a week in line with national and local standards.
  • Work collaboratively with senior managers to support Directorate and Divisional objectives, ensuring strong clinical engagement and partnership working.
  • Promote and share best practice, supporting a consistent, evidence-based approach across the service.
  • Foster a culture of continuous improvement, identifying opportunities to enhance patient outcomes, strengthen pathways and support innovation.
  • Contribute to the clinical governance agenda, ensuring robust governance processes are in place and followed.
  • Participate in audits, incident reviews and complaints processes, ensuring learning is shared and embedded to improve safety and quality.
  • Work flexibly to meet patient needs, service demands and wider team requirements across Intermediate Care and Hospital at Home.
  • Engage with internal and external clinical networks, representing the service and supporting system-wide learning and development.
  • Support responsible use of resources, contributing to financial balance and cost-improvement plans.
