Band 3 - 34 hours per week

An exciting opportunity has arisen for a dynamic individual with a good understanding of and skills with information technology to join the Integrated Diabetes Service, supporting people living with diabetes in the Tameside and Glossop Area.

Do you have a passion for diabetes and an interest in working within an integrated diabetes service?

The successful candidate will be working within a multi-professional service alongside Diabetes Specialist Nurses, Consultant Diabetologists, and a Diabetes Specialist Dietitian.

The successful candidate will be required to complete a series of competencies related to their role, in order to support development and identify learning needs.

Please note that whilst the trust does offer sponsorship this role isunfortunately not eligible for visa sponsorshipunder the UK Health and Care Worker visa route. This is due to the position not meeting theminimum salary threshold of £25,000 per annum and the required skill levelset by UK Visas and Immigration.

Main duties of the job

The post holder will be required to support the Diabetes Specialist Nurses and Consultant Diabetologists to deliver community and inpatient provision for diabetes care, and will have the opportunity to work with a range of patient groups, including community clinics, specialised clinics, young persons, insulin pumps and wards.

Key duties will include:

Managing clinic waiting areas; Greeting patients on arrival to clinics, ensuring they have been booked in, being a visible point of contact for patients waiting to be seen in clinic and assisting with keeping patients updated as to Diabetes clinic running times (if applicable).

Undertaking patient vital signs as part of pre-appointment checks; including: Weight, height, Blood Pressure, Pulse.

Perform and record blood glucose and ketone levels, utilising Trust point of care blood glucose monitoring equipment.

Perform and record point of care HbA1c testing, using appropriate equipment (training will be given if required).

Recording all undertaken observations promptly into the patient electronic record (EMIS)

Undertake venepuncture on patients as directed by registered healthcare professional.

Escalate any concerns encountered immediately to the appropriate registered healthcare professional on duty.

Support the Diabetes Specialist Nurses in the delivery of patient education and staff education and support.
