An exciting opportunity has arisen for a Registered Nurse Associate/Assistant Practitioner to join our Community IV Therapy Service.
We are looking for a highly motivated and dynamic individual to undertake the role of Service Co-Ordinator and Assistant Practitioner.
The successful candidate will undertake a dual role, in the first instance, co-ordinating day to day running of the service supported by administrative and clinical support personnel. In the second instance as a skilled assistant practitioner/Nurse Associate, performing clinical duties in line with vascular access, pathology sampling and supporting Senior Nurse Specialist in advanced vascular access device placement
Main duties of the job
- Co-Ordinating hospital discharges to ensure a seamless flow from the hospital to community
- Reviewing of pathology reports in line with service pathways and esclalting treatment plans as required
- Ensuring prescribed medicines are arranged in a timely manner.
- Maitaining of stocks and consumables required for service provision.
- Provide effective communication to support positive patient outcomes
- Venepuncture and cannulation skills
- Administer Intramuscular Medication
- Monitor and record vital signs in line with NEWS2 and Sepsis screening
- Support and assist in Ultrasound guidance vascular device placement
