Our Healthy Child Programme 0-19 (25 SEND) Service for Health Visiting and School Nursing has a vision to improve health outcomes for our population and influence the wider determinants of health, through collaboration with the people of Tameside and Glossop and our health and care partners.We are currently progressing through an exciting service transformation in conjunction with all of our team.

We work in neighbourhood models which align with these other services to best meet the needs for the community. Our visible Leadership team is positive in driving service improvement in a compassionate and collaborative way working with all teams to ensure we deliver high quality care in a supportive team environment.

You will provide compassionate, community-based support to children, young people, and families in Tameside in Greater Manchester, helping them stay healthy and avoid unnecessary hospital visits. You will be responsible for a caseload of children and deliver the Healthy Child Programme within a team. To ensure children receive the best start in life and school readiness is promoted.

Main duties of the job

You willlead and manage the delivery of the Healthy Child Programme, work with complex cases including safeguarding. Identifying at the earliest opportunity children with Special Educational Needs and Disability - signposting and supporting children, young people and families whilst being leaders in public health. You will work in collaboration with our neighbourhoods, communities, voluntary and statutory agencies. We deliver the Greater Manchester Early Years Delivery Model and are committed to improving our school readiness rates.
