Palliative Care Health Care Support Worker Band 3 - 33.5 hours per week
A great opportunity has arisen for a Health Care Support Worker to join our community Palliative Respite Team.
The post holder will work as part of the Palliative Care Respite Team and will be responsible to the Team Coordinator. The Post Holder will assist the District Nursing Team in the delivery of high quality community based nursing and social care to patients who have palliative care needs and who wish to remain at home. The post holder will work collaboratively with other members of the Intermediate Tier Services in supporting the patient and carers and provide personal, social, psychological and spiritual care. They regularly work without direct supervision in a patient's home.
Main duties of the job
Delivers nursing and social care to patients who have a palliative diagnosis and who wish to remain at home under the direction of the palliative care respite team coordinator and under the supervision of the district nurse. This will be in the form of personal care, ie, pressure relief, personal hygiene, continence care, oral hygiene etc.
To provide social support and care, ie to prepare drinks and light meals for the patient if required. To provide a flexible sitting and support service to enable carers to have respite periods and maintain patients within theirown homes.
