An exciting opportunity has arisen within the Division of Intermediate Tier Services,we are seeking an Enhanced clinical Practitioners to join the Extensive Care service.
It is an exciting time to join our team as we are part of a fully integrated care organisation and work acrossfive Neighbourhoods in Tameside and Glossop.
Extensive Care is an Umbrella term as the service has a number of sub services within it.
TheExtensivist service is well established over 5 years, and sees patient in the community who have multiple hospital attendances, along with long term conditions and complex health needs. We are really excited to be also supporting and developing our acute frailty service, with an ambition for an acute frailty path way into the community.
The service main focus is toreduce unnecessary hospital attendances, treating patient at home where clinically safe to do so, whilst providing exceptional standards of care.
This is an excellent opportunity for a visionary Enhanced Clinical Practitioner, to support systems towards inclusive compassionate care for all, delivering improved outcomes for Tameside and Glossop's diverse communities.

Main duties of the job
Main duties of the job

Registered Nurse / paramedic / Therapist with a current registration
-
working at an autonomous level
-
Independent non-medical prescriber
-
Clinical examination course desirable

Car and driving licence essential

NMC Approved mentor preparation course or equivalent
The successful candidate will provide highly specialised care within the community and Care setting, to those patients with chronic and acute conditions using clinical knowledge that is evidence based
The Enhanced Practitioner will function as part of an integrated Care organisation and will demonstrate competence in advanced assessment, management and treatment.
Provide expert advice and to ensure the needs of the patient are met by leading, challenging and changing practice within the community.
Work collaboratively with other health care professionals in primary and secondary care to ensure a high quality easily accessible service.
Provide a community wide lead in the development of the role of the Enhanced practitioner and the service provided
To support new and follow-up referrals
To pro-actively support and maintain patients within the community and care home setting
To facilitate discharge of patients back to their place of residence as appropriate
To undertake clinical audit
