An exciting opportunity has arisen at the Tameside and Glossop ICFT to join our Colorectal Specialist Nursing Service. We want you to be a passionate and empathetic individual, with strong communication skills and experience of supporting people with cancer.
We are looking for an enthusiastic nurse to join our Macmillan Colorectal Nursing team to provide high quality care for our busy colorectal cancer service. The post holder will join a friendly and established team including a Band 7 Colorectal Clinical Nurse Specialist and two Associate Clinical Nurse Specialists. We have Consultant surgeons who perform laparoscopic surgery for colorectal cancer and minimally invasive surgery for early rectal cancers as well as providing diagnostic and therapeutic colonoscopies. Our Colorectal service provides nurse led rapid diagnostic 2 week wait and treatment pathways for colorectal cancers, patient centred nurse led colorectal cancer service for our patients including stratified follow up, holistic needs assessments and personalised care.
Main duties of the job
To work with the Colorectal CNS team, Colorectal Consultant team, MDT coordinators, Colorectal Pathway Navigator, and diagnostic service colleagues to achieve a best-timed Colorectal cancer pathway. To deliver evidence based, clinical diagnostic protocols. To use advanced communication skills in supporting patients throughout the diagnostic pathway; using advanced clinical assessment skills to ensure all relevant clinical information is available to the multi-disciplinary team. To use specialist nursing knowledge to review and interpret all clinical investigatory results and determine next pathway stage. Demonstrate a higher level of practice through the provision of specialist nursing knowledge and expertise in the delivery of diagnostic pathways, including LGI suspected cancer. Establish and maintain robust communication channels with professional colleagues across a range of services. To be self-directing and work autonomously, identifying and maintaining supportive networks, and escalate as appropriate. Supervise junior nurse colleagues working within the colorectal service, adhering to Trust HR processes and staff development. Support the Trust in achieving compliance with Urgent Suspected Cancer (USC) and Referral to Treatment (RTT) waiting time targets.

- Responsible for provision of a high quality service delivering evidence-based, individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions.
- Supporting the colorectal best-timed pathway alongside the wider Colorectal CNS team, Consultant team and Pathway.
- The CNS will support patients in clinic or on wards when being given a new diagnosis of a Colorectal cancer.
- The CNS will attend the Sector Multi-DisciplinaryTeam (SMDT) when appropriate to represent Tameside patients or act as the link between the SMDT and the local teams ensuring the SMDT outcomes with Colorectal cancer patients are actioned.
- Act as a Key Worker for patients with Colorectal cancer, liaising between multi centres of care.
- The CNS will implement personalised care by carrying out eHNAs (electronic holistic needs assessments) as appropriate via an outpatient HNA clinic, and also help to facilitate health and wellbeing events for this patient group.
- Support patients in accessing, interpreting and appraising complex technical information to help them determine their future.
- Assess patients and their families information needs and provide relevant information to meet those needs. Effectively utilise appropriate information in accordance responding to communication barriers such as language or vision.
- Provide psychological support to patients at critical times of times of their illness pathway and during times of increased distress.
- Assist with symptom control and liaise with palliative care colleagues where appropriate.
- Assist with basic benefits advice or refer to welfare rights.
- Refer and liaise with community nurses, GPs and Willow Wood Hospice as needed.
- Work collaboratively with different teams and disciplines to improve patient pathways and individual patient outcomes.
- Actively promote self-management, undertaking risk stratification to determine those who can self-manage with education and support.
- Provide expert clinical assessment, care planning and evaluation, along with clinical advice, in accordance with NMC Code of Professional Conduct.
