Service Development Practitioner Band 8a

The post holder will lead service improvement and productivity projects & programmes from inception to conclusion, delivering real improvements in patient safety, patient experience, clinical effectiveness and productivity.

A key responsibility of the role will be to drive improvement & innovation work by challenging current clinical practice and by working teams to identify opportunities for better quality of service while delivering greater productivity. The post holder will be required to use a number of recognised quality improvement approaches.

The post holder will encourage innovation and an appetite for change amongst staff, leading by example by constantly challenging current practice and identifying opportunities for change from previous experiences and good [practice from elsewhere.

The role will support the operational teams in community settings, focusing on a whole system, integrated approach to patient care, whilst facilitating cultural and behavioural changes.

Main duties of the job

  • To support the development and implementation of new models of care across the health (Acute & Community) and social care system to support the specific transformation programmes and projects aligned to the delivery of local strategic plans.

  • To lead change projects that support the organisation in achieving its performance through implementing a range of improvement methodologies in line with the Trust's service/business objectives in order to promote sustainable change.

  • To provide expert leadership and service re-design, enabling the development of improvement initiatives that will lead to improved efficiency and deliver quality care to patients and service users.

  • Develop medium/long term service improvement objectives which are driven by local and national priorities, patient care, patient experience, and robust information, evidence of capacity and demand analysis and clinical governance requirements.

  • Lead and manage improvement initiatives from their inception to successful implementation. This will include the formulation of project plans and reporting structures, agreeing improvement methodologies and the monitoring of progress against agreed milestones and outcomes.
