Advanced Pharmacist - Speciality Medicine - Band 8a
We are seeking an exceptional and suitably experienced pharmacist for this post to shape and develop the delivery of the Speciality Medicine agenda for Tameside and Glossop ICFT.
You will ensure that patients receive safe, high quality and clinically effective pharmaceutical care whilst at the same time promoting evidence based, cost effective prescribing.
Our Pharmacy department has a strong clinical presence in all areas providing a much valued contribution to patient care, with a track record of achieving quality, operational and financial targets. We are a friendly innovative and forward thinking department

You will represent the pharmacy department locally at safety, quality, governance and business meetings and help to ensure that all medicines related activities comply with local and national policies and guidance
The post holder will be required to participate in the Pharmacy department's on-call rota and work extended hours and weekends/bank holidays on a rota basis.

Main duties of the job
To work within the clinical pharmacy team and lead the provision of a dedicated, patient focused, clinical pharmacy service for the Specialty Medicine element of the Division of Medicine and Urgent Care at Tameside and Glossop Integrated Care NHS Foundation Trust (T&G ICFT).
Work with the Advanced Clinical Pharmacist - General Medicine to ensure robust medicines management governance throughout the Division of Medicine.
To lead the Satellite Pharmacy Service Provision throughout the Trust.
To be the primary link between the Pharmacy Service and the T&G ICFT bed management team.
Produce reports and present clinical and financial information to the relevant specialty meetings and overarching Divisional meetings.

SECTION 1- MAIN RESPONSIBILITIES:
- Lead the pharmaceutical service elements to the Speciality Medicine Directorate patients of T&G ICFT, including:
- Gastroenterology,
- Diabetes/Endocrinology,
- Dermatology,
- Acute Oncology,
- Neurosciences,
(Acute Oncology, Neurosciences and Renal services are provided to the Trust via SLAs with local providers)
- To be the primary link between the Pharmacy Service and the T&G ICFT bed management team ensuring any issues affecting patient flow which are within the control of the Pharmacy service are resolved in a timely fashion.
- Liaise with Intermediate Tier services as needed to ensure safe, timely discharge/transfer of patients.
- Advise medical, nursing and other staff on the efficient prescribing, administration, storage and security of medicines.
- Develop regimens, PGDs, policies and protocols relating to clinical areas within remit.
- Participate in multidisciplinary team meetings as appropriate.
- Support the Speciality Medicine elements of the Division of Medicine and Urgent Care in provision of information on the use of medicines within each speciality.
- Represent the Pharmacy Service in areas pertaining to the role at a regional and national level as appropriate.
Clinical Pharmacy and Medicines Optimisation:
- Provide an advanced clinical pharmacy service to Speciality Medicine ward areas in accordance with professional, departmental and trust policies. This will include:
- Compiling a drug history where possible
- Reviewing prescribed medication to ensure safe and effective use.
- Formulating individual pharmaceutical care plans.
- Reviewing and updating care plans depending on clinical response, results of investigations and adverse effects.
- Interpreting and requesting clinical tests to monitor and make recommendations to improve effectiveness of drug therapy.
- Keep abreast of new drug developments and contribute to new drug submissions to the Integrated Medicines Optimisation Group (IMOG) in order to ensure that patients receive safe and effective treatment.
- Undertaking therapeutic drug monitoring for specific medicines.
- Developing evidence based treatment protocols for use across the Trust where appropriate.
- Facilitating implementation of local and national guidelines where appropriate.
- Liaising with medical and nursing staff to ensure medication is prescribed and administered correctly in accordance with Trust policy, to maximise patient safety.
- Investigate medicines related incidents or complaints and explore ways of minimising risk of future recurrence or risk to staff or patients. Complete clinical incident report forms where appropriate.
- Undertake clinical and practice audits within the pharmacy department and in clinical areas
- Liaise with appropriate health care professionals within the Clinical Commissioning Group, Primary Care Network Pharmacists and Medicines Optimisation in Care Home Pharmacists to ensure the effective and efficient use of medicines across the health economy
- Participate in ward and clinical pharmacy services and provide advice to ensure safe and cost effective prescribing and drug expenditure control.
- Monitor in-patient and discharge prescriptions for clinical accuracy and cost effective prescribing.
- Assess the patients response to therapy, interpret blood results, and depending on their clinical parameters either adjust drug doses in accordance with local procedures or advises the clinicians appropriately.
- Advise on therapeutic drug monitoring as appropriate to ensure patients receive optimum and effective treatment.
- Monitor and report Adverse Drug Reactions that patients experience as a result of their medication in line with national guidance.
- Participate on consultant ward rounds where applicable.
- Document clinical interventions and activities in line with local policy.
- Participate in specialist inpatient and outpatient clinical pharmacy services in accordance with local provision and work with healthcare staff to ensure the efficient operation of these services.
- Use inter-personal skills to counsel patients to ensure they gain maximum benefit from their medicines, and educate them regarding the use of equipment for the administration of medicines.

- Vocational Masters degree in Pharmacy (4 years) or recognised equivalent
- Pre-registration training and experience
- Registration with General Pharmaceutical Council
- Completed or undertaking a post-graduate diploma / MSc in clinical pharmacy (or equivalent experience)
- Post registration experience
- Working in a busy hospital (or equivalent documented clinical experience)
- Specialist and extensive knowledge of therapeutics of common disease states and frequently used therapeutic regimes
- Skills for analysing prescribing documentation , clinical systems , and complex patient , medicines or financial information in order to provide advice on medicines and expenditure
- Excellent and effective communication and interpersonal skills both verbal and in writing with patients and service users
- Self-motivated , Able to work on own initiative and as part of a large team
- Able to prioritise and organise own workload
