Critical Care Outreach Practitioner Band 7 - Permanent roles 1 x 37.5 hours 1 x 33 hours

The Critical Care Outreach Team (CCOT) is delighted to offer an opportunity for a registered nurse with recent experience in critical care or an acute/emergency care background to join our team.

CCOT is a 24/7 team supporting the Trust to safely and effectively manage our deteriorating patients.

Main duties of the job

The outreach practitioner will help facilitate discharges from Critical care to the wards, providing clinical training to ward staff, and acting as role models and mentors for ward staff. They will also undertake follow up of critical care patients post discharge from Intensive care. The Outreach practitioner will ensure that their skills are maintained by undertaking clinical shifts within the Intensive care unit.

  • First level registered Nurse with current UK registration (NMC)
  • Educated to Degree Level in relevant subject
  • Senior band 6 in an acute care setting e.g. A&E/Critical Care
  • Experience in the delivery of clinical teaching /formal teaching sessions
  • Excellent clinical reasoning/clinical decision making skills
  • Evidence of strong leadership skills
  • Evidence of well-developed scope of practice
  • Extensive clinical knowledge in the management and care of the acutely ill patient

Desirable

  • ALS provider/ Willingness to undertake
  • Post registration qualification in an acute specialty
  • Transfer Training
  • AdvDip Clinical examination Skills/willingness to undertake
  • Extensive experience in managing acutely unwell patients Level 2&3
  • A Clear vision of the scope of the outreach service
  • Evidence of change management/Project management
