Advanced Pharmacist-Education and Training
Band 8a - 37.5 hours - permanent
Due to significant investment in our Pharmacy service we are seeking an exceptional Pharmacist for this post to shape and deliver Education and Training for our Pharmacists at Tameside and Glossop ICNHSFT.
Main duties of the job
- To coordinate Trainee pharmacist training (with the support of Trainee Pharmacist Tutors)
- To tutor diploma students
- To actively organise, participate in & contribute to Pharmacy clinical meetings
- To provide medicines optimisation and teaching/training on prescribing to Pharmacy/ Nursing staff & Foundation doctors, with a particular focus on Acute Kidney Injury.
- To act as a professional role model and mentor in clinical pharmacy practice to junior Pharmacists and Technicians.
- This person would be expected to support on-call,late night, weekend and bank holiday working.

MAIN RESPONSIBILITIES:
Education & Training
- To coordinate Trainee pharmacist training (with the support of Trainee Pharmacist Tutors).
- To tutor Diploma students.
- Actively organise, participate in & contribute to Pharmacy clinical meetings
- To provide medicines optimisation and teaching/training on prescribing to Pharmacy/ Nursing staff & Foundation doctors, with a particular focus on Acute Kidney Injury.
- Keep abreast of current Pharmaceutical practice & knowledge, attending meetings, courses or conferences as necessary for Continuing Professional development.
- To act as a professional role model and mentor in clinical pharmacy practice to junior pharmacists and technicians.
Clinical Pharmacy and Medicines Optimisation:
- Provide an advanced clinical pharmacy service to ward areas in accordance with professional, departmental and trust policies. This will include:
- Compiling a drug history where possible
- Reviewing prescribed medication to ensure safe and effective use.
- Formulating individual pharmaceutical care plans.
- Reviewing and updating care plans depending on clinical response, results of investigations and adverse effects.
- Interpreting and requesting clinical tests to monitor and make recommendations to improve effectiveness of drug therapy.
- Undertaking therapeutic drug monitoring for specific medicines.
- Investigate medicines related incidents or complaints and explore ways of minimising risk of future recurrence or risk to staff or patients. Complete clinical incident report forms where appropriate.
- Monitor in-patient and discharge prescriptions for clinical accuracy and cost effective prescribing.
- Assess the patients response to therapy, interpret blood results, and depending on their clinical parameters either adjust drug doses in accordance with local procedures or advises the clinicians appropriately.
- Monitor and report Adverse Drug Reactions that patients experience as a result of their medication in line with national guidance.
- Document clinical interventions and activities in line with local policy.
- Use inter-personal skills to counsel patients to ensure they gain maximum benefit from their medicines, and educate them regarding the use of equipment for the administration of medicines.
- Promote adherence to the hospital formulary, treatment guidelines and protocols.
- Assist in drug utilisation and clinical audit activities.
- Initiate investigations into problems arising from the use of pharmaceutical products and assist in the development of new methods of service delivery.
Managerial:
- With appropriate training, supervise Band 7 and Band 6 Pharmacists; conducting appraisals, sickness and absence and performance reviews.
Dispensary:
- Participate in dispensing services including clinical and final accuracy checking of prescriptions / orders for inpatients, outpatients, patients being discharged and mental health patients in accordance with local standards, policies and safe systems of work and to be professionally / legally responsible for this work.
- Supervise work carried out by other pharmaceutical staff as required ensuring compliance with agreed legal and professional standards.
- Counsel outpatients to ensure that they gain maximum benefit from their medicines.
Ward Distribution Service:
- Liaise with other pharmacy staff providing a supply service to wards in order to maintain safe and economic stock levels. Ensure appropriate storage of pharmaceutical products on wards and departments and that these products are suitable and fit for use.
- Undertake or take responsibility for ensuring that the Controlled Drug stocks on Critical Care are checked every 3 months.
- Provide safe and secure transport of medicines to and from the wards as necessary.
Medicines Information:
- Provide information and advice on all aspects of pharmaceutical products as requested by medical & nursing staff and other health professionals including other Trust professional staff, GPs, community pharmacists, and residential care and nursing home staff.
- Provide information and advice to patients on all aspects of their medicines
- Undertake critical appraisal and evaluation of literature relating to the use of pharmaceutical products.
Education & Training:
- Provide individual assessments and testimonials for Trainee pharmacists as evidence of their training
- Undertake mandatory training in accordance with Trust requirements
General Duties:
Maintain a high standard of work and promote the professional image of Pharmacy within and outside the department.
- Ensure that safe working systems are in operation in accordance with Trust and departmental procedures.
- Ensure that all legal requirements relating to the prescribing, dispensing and storage of pharmaceutical products in the Trust are met and are in accordance with criteria set out under COSSH regulations.
- To participate in On-call*, Weekend, Bank Holiday and extended hours duties
*Participates in the provision of the on-call service (as requested) in accordance with local procedures. This involves the provision of advice and information relating to the treatment of patients and the supply of urgently required pharmaceutical products outside of normal working hours. This may require arranging supply of pharmaceutical products from other hospitals or wholesalers or taking appropriate action following receipt of Drug Alerts outside of normal working hours in accordance with local procedure.
- Hold a set of keys for the pharmacy department with associated responsibility for the security of drugs and equipment.
- Undertake manual handling up to maximum of 15Kg per item regularly each day
