Primary Care Network Pharmacist - DentonBand 7 -30 Hours Part Time - Permanent

The PCN Pharmacist will work as part of the PCN multidisciplinary team to deliver and develop all elements of programmes which involve the optimisation of medicines for the patients of Denton

Working within a team of PCN Pharmacists, you will work collaboratively with colleagues and the Advanced PCN Pharmacist to ensure that pharmacy services are delivered to the highest possible standards.

Across Tameside and Glossop we have the benefit that our PCN teams do not work in isolation and support is offered from other Primary Care Network Pharmacists across the conurbation as well as the Hospital Pharmacy service.

Main duties of the job

The PCN Pharmacist will work as part of the multidisciplinary primary care network team to deliver all elements of programmes which involve the optimisation of medicines.The PCN Pharmacists are responsible for working with their assigned GP surgeries delivering and developing a clinical pharmacy structured medication review service to patients in the community, in particular those identified by the T&G Public Health risk stratification tool as being at high risk of hospital admission or otherwise vulnerable. This may also include patients in care homes.

SECTION 1- MAIN DUTIES AND RESPONSIBILITIES:Foundation Level Implement policies and effective processes for medicines governance including controlled drugs Implement effective systems for monitoring high risk medicines Work with the Antimicrobial Pharmacists to ensure effective antimicrobial stewardship Ensure Medicines Policies and procedures meet the requirements of the Care Quality Commission (CQC) Integrate the GP Practice with local health and social care teams including Hospitals and Care/ Nursing homes Ensure safe, effective and timely medicines reconciliation at transfer of care (supported discharge from Hospital or Intermediate Care) Use software solutions to monitor and improve prescribing and to identify and prioritise patients requiring structured medication reviews Participate in national and local Quality, Innovation, Productivity and Prevention (QIPP) projects Identify topics for local quality improvement projects and participating in those projects Facilitate education and training for staff, patients and carers on medicines related topics Deal with medicines-related queries from patients, carers, colleagues and other PCN healthcare professionals and ensure responses are tailored to the patients medical, social and cultural needs Perform structured medication reviews for ambulant, housebound and care/ nursing home patients Use GP prescribing systems to ascertain relevant patient clinical data to support structured medication reviews, and being proactive in ensuring all relevant patient information is obtained prior to patient review. Intermediate Level Establish a programme of regular structured medication reviews for targeted patients Develop individualised treatment plans for patients taking multiple medicines, particularly those at risk of admission or readmission to Hospital Promote self-management for patients with one or more long-term conditions Care for and support patients taking high risk medicines Care for and support patients with specific clinical conditions, including but notrestricted to: patients with hypertension, atrial fibrillation, coronary heart disease, heart failure, asthma, COPD, diabetesAdvanced Level Develop an area of clinical specialism in order to lead as a local medicinesoptimisation specialist in that clinical area Prevent medicines-related falls by carrying out structured medication reviews Optimise medicines for patients with multiple long term conditions Review complex medication regimes including making suggestions about deprescribing where appropriate Care for older and frail patients, including those with dementia Advise on medication required for end of life care Reduce inappropriate polypharmacy and waste through clinical medication review and medicines optimisation and promote cost-effective evidence based prescribingWorking Across Interfaces Facilitate communications between GPs, hospital doctors and pharmacists, community pharmacists and community nurses to ensure all key stakeholders are aware of specific patient medication related issues and the actions required to optimise individual patients pharmaceutical needs.
