The Integrated Urgent Care Team is a joint health and social care service which is leading on the implementation of 'Home First' which is Tameside and Glossop Admission Avoidance and Discharge to Assess Model.

The first part of our Urgent Care service transformation will be based around The Integrated Urgent Care Team (IUCT). The service strives to provide high quality community healthcare services across Tameside & Glossop. Main duties of the job

To help us achieve our ambitions we are seeking to recruit a Band 6 Physiotherapist 37.5 hours per week , with the skills to maximise independence and support people to remain in their own homes. These positions all include daytime, evening and weekend shift patterns. The current shift times are carried out over a seven-day rota including weekends, bank holidays, between the hours of 8:00am - 10pm. Core hours may be subject to change as the planned service developments are taken forward.

The successful candidates ideally would have previous community experience, relevant clinical skills with an enthusiasm and commitment to continuing professional development. You must be able to work alone and without direct supervision, within your agreed scope of clinical competence. You will have excellent communication and organisational skills. Previous experience or knowledge of working in a multi-disciplinary team will be an advantage for this post.

As the positions will require visits to various locations throughout the localities, it is essential that candidates hold a driver's license and have use of a car for business purposes to commute within the Trust's geographical area to ensure a timely delivery of patient care.

They will undertake a leading role in the assessment, diagnosis and formulation of therapy treatment goals as well as advising and treating patients, carers, families and staff of other agencies. The post holder will also provide daily leadership within the Multi-Disciplinary Team (MDT) with other therapists, nursing staff, Technical Instructors, Health Care Therapy Assistants, students and other health professionals visiting the unit.
