Job summary

The vision for an Integrated Discharge Team in Bolton is for the three main stakeholders, the Trust, the Council and the ICB to work in partnership to provide a fully integrated and co-ordinated hospital discharge service for patients admitted to wards in Bolton NHS Trust. The post holder will play a key part in facilitating timely and safe discharges for patients, and liaising with relevant social and NHS staff to support this.

Main duties of the job

The post holder will support the Senior Leadership Team (SLT) in the Integrated Discharge Team, by assisting the team leads to facilitate timely discharges for patients, and ensuring that efficient and effective processes are in place to prevent delays in discharge. The post holder will attend Multi-Disciplinary Team Meetings and liaise with all relevant health and social care professionals and managers, to ensure patient flow and hospital discharge/transfers of care are safely and effectively managed.

  • oPrevious experience in completing patient documentation
