Job summary

Part time 30 hours per week. Permanent post.

The Deputy Bereavement Officer will be the first point of contact for hospital deaths and will guide and support the Bereaved family through the death certification process . Working with the medical examiners, coroners, medical staff and Bereavement Nurses .

You will part of the Bereavement and Patients services team dealing with all hospital death .

The Team are lead by the clinical service lead for Bereavement and organ donation and the Bereavement Nurse working very closely together to support bereaved families .

Job description

Job responsibilities

1.The Post holder will be the key contact for bereaved family relatives to support them through the process of arranging and registering when someone dies in the hospital.

2.The post holder will assist with the preparation and documentation of death certificates, working with medical staff and medical examiners this will also include supporting referrals to coroners and supporting funeral planning.

3.The post holder will support the Bereavement officer in arranging funerals for adults and still births when necessary liaising with bereaved families

4.They will support the efficient channelling of communication with medical examiner office , bereaved families, discussions with the Registers, coroners department staff , other organizations members of the public and other disciplines within the Trust,

5. Support Witnessing of Wills, Court of protection and third party mandate forms for inpatients

6. Complete claims forms for patients and families and assist with Department of Work and Pensions enquires regarding admission and discharge dates either by telephone or letter

7. To receive valuables from the ward, to place in the safe ,and return valuables when patient discharged or has died.

8. To support maintenance of patient property accounts liaising with finance

9. To deal in a professional, helpful and sensitive manner with families, staff and other agencies by telephone or face to face, taking messages, advising families about appointment , referral to the Coroner, medical examiner office and Registrar

10. Receive notification of Road Traffic Collisions compensation claims form, verify if treatment has been received or not either as in patient or outpatient by the Trust

11. Complete RTA2 forms, indicating number of days in hospitals and return forms to Compensation recovery unit

12. Liaise and communicate with Compensation Recovery Unit, A&E , Health records and Divisional manager to resolve discrepancies

13. Advice finance manager and Clinical Service Lead regarding monthly amounts cancelled and withdrawn, verifying the amount paid by Compensation recovery unit on monthly pay list

14. To manage and priorities workload on a daily basis and deal with the competing demands

15. The post holder will demonstrate excellent organizational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for compassion, confidentiality and integrity.

16.To be proficient in Computer systems EPR . LE2.2 tracking case notes and complying with Data Protection Act and Information Governance

17. To maintain confidentiality at all times when dealing with information and sensitive Trust information
