Secondment Opportunity. Part-time 30 hours per week
The role of the Vascular Practitioner in a network hospital is key to ensuring the service provides the best care for Bolton patients who will undergo surgery at Manchester Royal Infirmary. The Vascular Practitioner will run nurse led clinics, pre-operative assessment clinics, exercise programme sessions and will also attend consultant clinics.
The post holder will be expected to work closely with colleagues fromboth organisations to ensure a seamless pathway for patients. This will involve liaising with consultants, specialist nurses and anaesthetists at the hub hospital.
The post holder will be expected to monitor standards of care through audit and evidenced based practice. She/he will be expected to participate in clinical governance within the service by undertaking audit and research as part of the MDT
Main duties of the job
- Provide advanced levels of clinical practice related to vascular conditions
- Carry out pre-operative assessments on patients due to have procedures and/or surgery both at the hub hospital and the spoke hospital and liaise with the surgeons/anaesthetists if any concerns
- Prescribe within scope of practice if indicated
- Offer health promotion to patients in order to reduce risk factors for disease progression
- Assist in the development of care pathways to develop a high quality seamless service based on the principals of best practice.
- Work autonomously to manage own caseload of patients whilst working as part of the Multidisciplinary
- Be accountable for all aspects of care, advice and treatment offered to patients within
- Detect and recognise potential problems in the patient's condition and take appropriate action to rectify. Advise other health care professionals in correct action and plan of
- Order diagnostic tests, interpret and act appropriately on
- Communicate highly sensitive and emotive, condition related information to patients and relatives. g. need for amputation of lower limb.
- Act as the patient advocate when relevant and be able to educate on all the treatment options to enable a patient to make an informed choice, negotiating with the medical team to agree a pathway of care

The role of the Vascular Specialist Practitioner in a spoke hospital is key to ensuring the service provides the best care for Bolton patients who may be admitted to Bolton or prepared for surgery at Manchester Royal Infirmary. The post-holder will be fully supported by the vascular Consultants serving Bolton patients, and the administrative infrastructure at Bolton, attending Vascular clinics with the surgeons, managing preoperative pathways and monitoring inpatient care pathways. The role is expected to develop to include nurse-led clinics and interventions at a rate commensurate with the experience and ability of the successful applicant. There will be full peer support from Vascular Specialist Nurses and other specialist practitioner groups within the MUFT service, including opportunities for professional meetings and attending clinical sessions on the Central Manchester site as indicated.The post holder will be expected to work closely with colleagues from both organizations to ensure a seamless pathway for patients. This will primarily involve liaising with the consultants and junior surgical team from the hub, both when they are working on the Bolton site but also by electronic communication when needed. Similarly, specialist nurses and other clinical groups at the hub hospital will be available for support. The post holder will be expected to monitor standards of care through audit and evidenced based practice. She/he will be expected to participate in clinical governance within the service as part of the MDT, and may take on audit/governance projects as required. It is expected that the post-holder would take up full or affiliate association with the Society of Vascular Nurses, as appropriate.All applicants must have significant experience within the health service and some experience of working within vascular services or pre-operative assessment would be advantageous.
