Job summary

This is an exciting opportunity to join a friendly, well established, integrated learning disability team with a focus on delivering high quality service for adults with learning disabilities. We are looking to appoint an enthusiastic occupational therapist to work with adults who have a learning disability and complex physical health needs and/or behaviours that challenge services. Bolton's learning disability health service is integrated into the Community Learning Disability Team, combining the learning disability health services with social care. The team is based at Farnworth Town Hall located within the Bolton borough. Bolton is within Greater Manchester and has good motorway links.

The post offers opportunities for the successful applicants to develop skills in meeting the complex needs of clients. The role will include housing assessment and adaptations, posture management, assistive technology and equipment provision.There will be opportunity to bring creative thinking into your work and use skills acquired throughout your previous Occupational Therapy training and experience. There will also be a leadership element to the role as the post holder will be responsible for the supervision and governance of the Occupational Therapy team.

The post holder will have access to clinical supervision, peer and managerial support. There is a commitment to providing ongoing training and professional development, including development of skills and leadership opportunities.

Main duties of the job

To provide clinical leadership to the Occupational therapy discipline under direct supervision of the AHP Lead focusing on quality improvement, clinical governance, and safe delivery of service.

To participate in collation and presentation of data to provide assurance on service delivery.

To support the Occupational therapists to manage their caseloads through supervision and appraisal, ensuring that they maintain an appropriate caseload.

Receive, record and allocate referrals to Occupational therapy within the specified timescales required.

In line with The Allied Health Professionals (AHP) Strategy for England, the post holder will support the implementation of policies designed to achieve quality in healthcare including championing diverse and inclusive leadership and improving representation from those with protected characteristics.

To participate in, clinical audits, effective use and monitoring of outcome measures, key performance indicators (KPI) and other initiatives to provide assurance of quality and value.

To deputise for the AHP lead when appropriate.

Job responsibilities

To be professionally and legally accountable for the management of an individual caseload. To work as an autonomous practitioner within the Disability Team as a member of a multi-disciplinary team of Health and Social Services personnel.

To provide occupational therapy to cover vacancies, sick leave, annual leave, as appropriate.

To prioritise and manage occupational therapy referrals and waiting lists to ensure clients receive appropriate and timely occupational therapy intervention in accordance with KPI objectives.

To be professionally responsible for the planning, co-ordination, delivery and evaluation of occupational therapy within the multi-disciplinary Team, prioritising clinical needs and available resources to provide an effective occupational therapy service.

To undertake highly specialist occupational therapy assessments for clients who have varying degrees of learning disability. The clients may also present with additional complex associated physical disabilities, complex health needs, communication difficulties, challenging behaviour, dual diagnosis and vulnerability.

To identify areas of occupational performance and skill deficit in all functional activities of daily life using a variety of assessment methods both standardised and non-standardised, advanced clinical reasoning skills and have a knowledge of how illness/disease affects function.

To work with clients, families and carers to identify client centred goals and work as part of a multi-disciplinary team to enable the goals to be achieved.

To carry out highly specialised and detailed risk assessments for clients with complex needs using local and national guidelines, professional guidelines and a highly developed knowledge base, taking lead responsibility for the overall safety and risk management of clients who are vulnerable and require on-going and long-term involvement with regard to management of specialised and bespoke equipment provision.

To plan and implement client centred evidence based individual/group interventions. This may include training and advising clients/carers, assessment and provision of specialist therapeutic equipment, providing training on independent living skills, risk assessments, advising and acting on environmental/housing issues and providing specialist occupational therapy knowledge within the team.

To continually monitor and evaluate client treatment in order to measure progress and when necessary modify programmes to ensure effectiveness of intervention.

To provide highly specialised advice and training to clients, relatives and carers regarding illness, disability and lifestyle management in order to maintain or achieve maximum levels of functional independence and well-being.

To be skilled in determining the clients understanding of occupational therapy assessment and intervention, to have the ability to gain informed consent from the client or, within a legal framework, work with clients who lack the ability to consent to treatment.

To be skilled in clear communication within highly charged, complex situations so that clients and their carers are able to work together, make informed choices and be involved at all stages of the intervention process.

To work as part of the Disability Team contributing to the effective sharing of written, electronic and verbal information to ensure the delivery of a co-ordinated multi-disciplinary service.

To work with multi-agency and external agencies such as Housing and Wheelchair Services to ensure that clients with a learning disability can access generic services in line with current legislation and NICE recommendations.

To provide support and empathy to clients and carers whilst having difficult conversations which enable them to manage complex lifestyle changes.

To communicate effectively with clients and carers using a range of verbal andnon-verbal communication tools to gain an understanding and consent with treatment programmes.
