Job summary
An exciting opportunity has arisen for an enthusiastic, motivated and experienced pharmacist to join our Community Services Pharmacy Team at Salford to support residents within care homes. The Care Home Pharmacist will undertake medication reviews, working in partnership with the Salford Care Homes Medical Practice, the wider MDT, care homes and secondary care. The Care Home Pharmacist will lead on medicines optimisation and provide support to achieve improvements in the quality and cost effectiveness of prescribing.You will need to be self-motivated and able to work on your own, whilst also integrating into established teams. You will hold a non-medical prescribing qualification (or be in the process of obtaining this qualification). In addition, you must have excellent organisational, interpersonal and communication skills.This is a fantastic opportunity to work collaboratively across primary and secondary care in an innovative service.
Main duties of the job
*This is a 9-month fixed term maternity cover*
Key responsibilities will include
Medicines reconciliation and structured medication reviews to highlight deprescribing and medicines rationalisation opportunities for care home residents
Participation in MDT meetings
Support with the transfer of information across secondary and primary care
Providing expert medication advice, education and training for members of the multidisciplinary team and care home staff
Working with key stakeholders across the interface of care to support with medicines related initiatives
Access to a vehicle for work purposes will be required in order to undertake this role
