The Northern Care Alliance are delighted to offer the opportunity to become our Lead Practive Nurse with The Northern Care Alliance NHS Foundation Trust.
The post holder will be responsible for delivery of safe and effective nursing care to the population of patients whom live in Nursing and Residential homes in Salford.
This is a service is cross the city of Salford.
As the Lead Practise Nurse the postal account for the nurses delivery service this will include leading and managing all nursing resources working closely with GP's and the practise manager to deliver a practise priorities.
The role incudes leading on the provision of specialist clinical nursing care, health promotion and chronic disease management to the patients registered with the practice. The post holder will lead on the delivery of the indicators in the Salford Standard, Quality and Outcomes Framework and NHSE KPI's ensuring the practice meet requirements.
Main duties of the job
Clinical Leadership & Patient Care
- Chronic Disease Management:Leading nurse-led service for long-term conditions, managing care plans, and performing reviews.
- Preventative Care:Delivering and leading immunisations, cervical cytology, and chronic disease .
- Clinical Expertise:Providing general nursing care, Fall assessment, wound management, and Ear syringing.
- Evidence-Based Practice:Implementing and evaluating care based on NICE guidelines and best evidence.
