Job summary

We have an exciting opportunity for a self-motivated, experiencedTherapist to join our A&E Therapy Team in OldhamCommunity.We need people who share our excitement, embrace different ways of workingand are able to work flexibly across organisational boundaries supporting the NHSLong Term Plan and admission avoidance agenda.PositionThe post holder will work as part of the multi-disciplinary team, driving thechanges to community care response to support the hospital avoidance agenda. They will work in line with the agreed strategic objectives of Northern CareAlliance.The aim of the team is to reduce unnecessary hospital admissions followingattendance to the Emergency Department, by providing holistic therapyintervention and a person centred approach support in order to enable patients toreturn to their own home or usual residence. The team works closely with community services to support the home first model.

Main duties of the job

The successful candidate will be expected to work independently, but also as anintegral member of the multidisciplinary team within the Emergency Department.The candidate will be able to demonstrate strategic planning and have a strongclinical background in Urgent Care or a similar field. Excellent time management and advanced communication skills are required along with awareness andunderstanding of local, regional and national agendas in respect to Health &Social care.
