Renal Young Adult Youth Worker

Job summary

This is a one-year part time (7.5 hours), Band 5 post for a Youth Worker (YW) to support the Renal Young Adult Clinic.

A YW offers an innovative approach to support medical and psychosocial wellbeing to young adults accessing renal care within Salford Care Organisation and the satellite renal services provided. In so doing, to improve health and social outcomes.

Main duties of the job

  • To participate as a professional member of the healthcare team and contribute to the care of the patients as part of the multidisciplinary team.
  • To act as a social prescribing link worker and enable access to local community services for appropriate recreational, social, educational, vocational and advocacy support where appropriate/on request of young person.
  • To contribute to the provision of a varied programme of activities and opportunities to engage young people, and support engagement with the national Young Adult Kidney Group.
  • To support young people transitioning from Paediatric to Adult Care.
