Job summary
We currently have an exciting opportunity to join our highly acclaimed and well-established Therapy Team based at the Bevan Intermediate Care Unit.
As part of a multidisciplinary team, you will help deliver and encourage our patients to participate in socially interactive activities.
You will identify a varied and interesting activity programme for our patients. You will document the engagement to activities by the service users and record in the electronic patient record (EPR).
You will ensure the activities available on the unit are appropriate for the patients. You will support alongside the therapy team to ensure patient fundamental care needs are met and dignity is maintained.
You will actively plan fundraising events for the ward with support from the therapy leads.
You will network with appropriate and relevant partners and agencies to promote healthy relationships between community and acute services.
You will actively lead publicising the activities and monthly campaigns on the ward with the Trusts communications team to ensure the profile of the unit is heightened.
You will contribute to the maintaining a safe and effective working environment being assigned duties from the Therapy Lead/Ward Manager.

Main duties of the job
With support from the therapy team, contribute to providing a range of social interaction and well-being activities, e.g., activity groups, and on an individual basis, with minimal supervision.
In collaboration with the patients and therapy leads maintain and develop the activity timetable for the unit and for each patient on an individual recovery focussed basis &alongside colleagues e.g., Occupational Health, Physiotherapy and Nursing staff to deliver a range of activities that are recovery focussed and suit the individual needs of each patient.
Undertake mobility, transfer, and activities of daily living practices, as delegatedby qualified therapists.
Document all engagement with clients in the patient electronic notes to ensure the clinical team is kept up to date with patient participation in activities.
Contribute to the regular updating of individual activity timetables for clients which offer a range of activities on an individual and group basis dependent on the client's needs. This can be subject to regular change.
Demonstrate safe and competent moving and handling skills, asking for support with complex cases. Undertake safe and competent equipment provision for stock items, asking for support with non-stock needs.
You will support learners in the workplace by providing an opportunity for shadowing with the activity co-ordinator to observe activities being planned and delivered.
