Join Our Pioneering Population Health Team as a Senior Nurse/AHP
Are you passionate about transforming healthcare and making a real difference in people's lives? We're seeking a dynamic Population Health Management (PHM) Senior Nurse/AHP to lead the charge in Oldham East's innovative health improvement initiative.
About the Role:You'll be a vital part of a forward-thinking team dedicated to proactive, preventative care for adults aged 50-64 who are at risk of frailty. Working across the PCN, you'll collaborate closely with a dedicated care coordinator to identify, engage, and support individuals early--helping them maintain their health, manage chronic conditions, and prevent deterioration.
What You'll Do:
- Drive early intervention strategies to improve health outcomes
- Provide clinical expertise directly to patients, ensuring personalised, compassionate care
- Work within a vibrant, integrated neighbourhood team alongside health and social care professionals, the NCA, and Oldham Council
- Contribute to a pioneering health program that puts prevention at the heart of care

Oldham East PCN is a vibrant alliance of six dedicated general practices committed to delivering exceptional healthcare across a diverse and expansive community:
- Lees Medical Practice
- Leesbrook Surgery
- Moorside Medical Practice
- Pennine Medical Centre
- Saddleworth Medical Practice
- Springfield House Medical Centre
In addition to their main sites in Uppermill and Mossley, Saddleworth Medical and Pennine Medical also serve patients through branch sites in Delph and Greenfield, ensuring accessible care across the region.
Our Community & Geography:Stretching from just outside Oldham town centre on the west, reaching up to Rochdale in the north, Tameside in the south, and into West Yorkshire in the east, the PCN covers a broad and diverse landscape. Notably, the Saddleworth area alone accounts for over half of the Metropolitan Borough of Oldham--approximately 54,000 residents--making it a vital part of our community.
