Assistant Practitioner - Community Stroke and Neuro Rehabilitation

Job summary

We have an exciting opportunity for a motivated and compassionate Assistant Practitioner to join our Community Stroke and Neuro-rehabilitation team within the Northern Care Alliance NHS Foundation Trust. You will work as part of a multidisciplinary team providing specialist rehabilitation to adults following stroke or neurological injury, supporting recovery and promoting independence in the community. Working under the supervision of registered professionals, you will deliver a range of delegated clinical and therapeutic interventions helping patients to achieve their rehabilitation goals in line with personalised care plans.We are a very supportive team and provide CPD and learning opportunities, training and supervision.

Main duties of the job

We're looking for an enthusiastic team player who shares our NCA values of Care, Appreciate and Inspire.

You will have completed the Foundation Degree in Health and Social Care (or Level 5 equivalent)

o Responsible for a designated caseload, undertaking therapy programmes as identified within their role and competencies.o With support, have excellent time management and communication skills to prioritise and balance clinical, patient related and service demands.o Contribute to multi disciplinary team meetingso Adopt an MDT approach in the management of the patiento Actively manage support workforce waiting list.o Contribute and support student education, senior therapy assistants and newly registered staff

Support the administrative needs of the service on a rota and ad hoc basis

Lead and develop links with Salford voluntary organisations to complement our intervention.
