Job summary
An opportunity has arisen for an experienced support worker to join an established team of anticoagulant nurses to provide support with monitoring patients who take Warfarin. You should be able to demonstrate the skills and knowledge of working within a busy multidisciplinary team setting but equally have ability to work autonomously. Working alongside the specialist anticoagulant nurses you will provide point of care INR testing for patients within a clinical setting both on hospital site and in the community. You will also be responsible for undertaking home visits for house-bound patients across Heywood, Middleton and Rochdale. You will plan your own routes each day and undertake INR testing following established clinical and lone working protocols.
The support worker will have the ability to travel around the HMR footprint and demonstrate experience of prioritising their own workload.
The successful applicant will have excellent communication skills to be able to convey patients' blood results with the anticoagulant nurse via telephone and leave a written instruction for the patient in their own homes. You will link in with community neighbourhood teams and Rapid response teams as necessary and be aware of community care pathways to facilitate caring for patients in their own homes.
The applicant will need to demonstrate a high level of autonomy and be caring and compassionate.
Main duties of the job
The post holder will assist the anticoagulant nurse in the running of hospital and community anticoagulant clinics and undertake tasks which have been delegated. You will undertake INR blood testing using Coaguchek near patient testing device and take responsibility for your own equipment, ensuring daily quality control and Standard Operating Procedures are followed.
You will independently carry out home visits for patients who are housebound which will involve planning and prioritising your visits to ensure good use of time management and fuel economy.
You will establish and maintain communication with patients, relatives and carers and the multidisciplinary team to be able to provide holistic patient care and be pivotal in assessing patients' health and wellbeing related needs in their own homes. Working flexibly between Primary and Secondary care you will help to provide specialist assessments to facilitate the management of housebound patients taking Warfarin.
