End of Life and Bereavement Nurse

Job summary

An exciting opportunity has arisen for a compassionate, motivated, and experienced individual to join our dedicated SWAN Bereavement Team within the Northern Care Alliance.

Working in alignment with the Swan model of care, you will provide discreet, empathetic support to individuals and families during the immediate stages of grief--regardless of age, cause, or place of death. This role involves education, support and empowering healthcare professionals in delivering practical and emotional care, including memory-making, guidance, and information before and after the death of a loved one

Our team offers compassionate presence and support to bereaved families, including those navigating the coronial process and attending inquests

The service also supports tissue donation processes and contribute to the I-Orbit Project in collaboration with NHS Blood and Transplant, including screening and initial family approaches for potential corneal and tissue donation. You will receive training in the tissue donation processes from NHS Blood and Transplant and the Bereavement Team.

This is not a counselling role, but one that offers a vital space for listening, understanding, and supporting people through the early stages of grief.

Main duties of the job

We're looking for a compassionate, motivated, and experienced individual who can work collaboratively with other professionals and agencies and in emotionally complex situations.

You will be a registered nurse with experience in palliative, end-of-life, or bereavement care and have excellent communication and interpersonal skills, with the ability to support families sensitively and professionally. With a commitment to compassionate care and continuous learning.

You will support the tissue donation processes including screening and initial family approaches for potential corneal and tissue donation and any related education to wards and departments.

The Bereavement Service is not a counselling service but can refer on or signpost families and carers if this level of intervention is identified. This can involve supporting those families going through the coronial process including those who need to attend an inquest.
