Job summary
The post holder will work across the Northern Care Alliance (NCA) be a part of the Learning Disabilities Acute Liaison Service providing highly expert knowledge.
Supporting the organisation to meets its responsibilities regarding adults, children and young people with learning disabilities, their families and /or Carers.
Main duties of the job
The Senior Learning Disability Liaison Nurse will contribute to the care and support for patients with a Learning Disability within an acute service for Northern Care Alliance.
You will be supporting patient with a diagnosis learning disability in accessing and ensuring the correct care and treatment processes. This includes the correct identification and flagging, advice and practical support regarding reasonable adjustments including any training opportunities.
You will be a senior member of the team and be required to work collaboratively with internal and external professionals. Including contributing to governance, audit and service development opportunities.
The post holder will work in partnership with patients, their families / carers, and health/social care professionals, to provide a specialist service for people with learning disabilities accessing Northern Care Alliance Hospital Services.
The post holder will be supporting and ensuring implementation of good practice from patients that access services via the emergency or planned admission routes. The post holder will also be expected to support outpatient conversations/ case discussions to support patients, carers, family and professionals.
The post holder will support to both Adults and Children with Learning Disabilities. This specific post will support patients of all ages.
