Job summary
Due to promotion, we're seeking a dynamic, experienced and highly motivated Pharmacy Technician to join our medicines optimisation care homes team at Salford.
Salford Care Homes Medical Practice is a unique GP Practice within Salford Care Organisation, providing medical care to older people living in residential and nursing homes within Salford. The successful candidate will work alongside the integrated multidisciplinary team at the Practice, participating in a range of medicines optimisation activities and supporting residents to get the best outcomes from their medicines.

In addition to working with our Care Home Pharmacists, you will have the support of our friendly community services pharmacy team. This is an exciting chance to learn new skills and develop your career, through tailored in-service training and leadership development opportunities.
Main duties of the job
This clinical role is based in a GP Practice, with visits to care homes across Salford for medicine reviews.Duties will include medication history taking, identifying and resolving medicines-related issues, answering telephone/email medicine queries from care homes, liaising with prescribers and community pharmacies regarding the supply of medicines and supporting pharmacists with post-hospital discharge medication reviews to reduce hospital readmission's for care home residents. Training of care home staff is an important feature of the role.
The post-holder will attend MDT meetings, providing advice to health and social care professionals. The post-holder will promote high quality and cost-effective prescribing and undertake audits.
As this is an ever-evolving role, the post-holder will contribute to new service developments and projects, to facilitate improvement and changes.
There is currently no weekend/late night commitment. Access to a vehicle for work purposes will be required
